Pre-Registration FAQs

Anyone who is scheduled for a service should pre-register. 

Pre-registration expedites your check-in process when you arrive at any of our hospital's. Also, as a service to you, Pre-registration allows us to ensure your insurance benefits are in place. If you are uninsured, Pre-registration allows us to present many financial options that may be available to you.

Simply complete the online pre-registration form, or call 404-459-1280. Our hours of operation are Monday – Thursday: 8:00 AM – 7:45 PM, Friday – Saturday: 8:00 AM – 4:45 PM. Closed Sunday. You will receive a confirmation e-mail after submitting your pre-registration form. Please print and complete the following forms and fax to 404-300-2333 or email ;the form along with copies of the front and back of your insurance card.

As soon as possible. Please understand that forms submitted fewer than three days prior to your service date may not be processed in time, potentially increasing check-in time processing. 

At Northside, your privacy is important to us. For your protection, all information submitted online is encrypted while in transit. We will not use your information for any other purposes. 

You will receive an email confirming that your pre-registration form has been received. 

When you present for your service or pre-testing, simply alert your clinician or registration representative about any changes. You may also call (404) 459-1280 or email your changes.

The estimate provided is based on historical averages and information obtained from your insurance company. Northside Hospital cannot predict the care you need and your physician orders. The final bill may differ substantially from the information provided, and Northside Hospital will not be liable for any discrepancies based on your actual, individual care provided. The estimate does not include professional fees (e.g., fees associated with the costs of Pathologists, Radiologists, Anesthesiologists).

Payment toward estimated patient responsibility is due prior to service.
